Registration and Practice Boundaries

Sycamore House does not discriminate on the grounds of:

  • race, gender, social class, age, religion, sexual orientation or appearance
  • disability or medical condition

Our practice boundaries are shown above, you can view, download and print a map here and a map showing our practice boundaries is also displayed in the main reception area. Anyone residing permanently within this area who wishes to join the practice should call into the practice and enquire about registration

  • You will be required to provide the details of your current doctor
  • We also recommend that you register for records access at the same time and bring the completed consent form and ID with you

Temporary registration is available to friends and relatives staying within the practice area

Anyone without a local GP needing to be seen in an emergency can be seen as an ’immediate and necessary treatment’ patient

Please note that if you register with us and you do not live within our catchment area shown above, you will be registered as an ‘out-of-area patient‘ and will not be entitled to any home visits from any of our doctors.
